Arizona family dental plans, looking to stay with our dentist

My wife and two kids live in Page AZ.  We need dental insurance since I changed my job but our dentist is not a provider of any plans.  So dental discount plans will not work.  We need dental insurance that will let us stay with our dentist but hopefully not cost  more then $100.00 a month if posible.  Please advise thanks
